Requirements for Entry Into Kindergarten
The required medical information indicated below must be provided to the school nurse prior to your child's entry into school. Failure to do so will result in your child's exclusion from school as required by Westford Public Schools' policy.
“No child shall, except as hereinafter provided, be admitted to school except upon presentation of a physician’s certificate that the child has been successfully immunized against diphtheria, pertussis, tetanus, measles and polio and such other communicable disease as may be specified from time to time by the Department of Public Health. “M.G.L. c. 76, & 15;105 CMR 200.000 section 15
1. Immunizations
Massachusetts School Immunization Law requires documentation of the following immunizations (unless there is written documentation of medical or religious exemption).
-DTAP (5)
-Polio (4)
-MMR (2)
-Hepatitis B (3)
-Varicella vaccine (2) - or documented date of chicken pox disease
2. Physical Exam
Physical exams are required for entry into kindergarten and for students transferring to Nabnasset School. They are also required in the 3rd, 7th, and 11th grades. Due to insurance constraints, a physical will be accepted for the previous year for children whose yearly exam comes in the fall (September- December). Please send in this year’s physical as soon as possible.
3. Lead
Documentation of previous blood lead screening test date and result is required by law upon entry into Kindergarten.
4. Vision Screening
Documentation of preschool vision screening is required by law upon entry into kindergarten.
